How to unlock the Co-op Win achievement in Golden Axe - Definitive Guide
This was my final Achievement to obtain in this game. Initially I was frustrated b/c I thought there would be no way to obtain without having someone to locally play the co-op mode with. There were two issues holding me back.
One, I had no one locally to play with. This can be avoided by simply having another local profile log in. This way, you can start the local co-op game itself. Once the game has started, all you need to do is have that profile not continue once defeated and you can carry on normally as you one in a single player game.
Two, I was under the impression you're only allowed a certain number of 'continues' before you'd get GAME OVER. Just as x2i4eva above me has pointed out, you're allowed an infinite amount of continues.
Therefore, you can get this Achievement without breaking a sweat. I will point out contrary to x2i4eva's statement that you can state save (like in single player) in co-op mode (local or live) there is no Save file available.

Legend of Josh is correct, you can do this using two controllers by yourself. What you want to do is start the game, and let the second controller die. Keep playing the game with your first controller straight through, and you'll get the achievement at the end. You can leave the default game settings (not sure if changing them impacts the achievement).
There is no need to have the second controller continue when it dies, as the game appears to only check at the very beginning of the match.

Grab a friend with an extra controller and simply beat the game - there is no danger of losing as there is infinite continues and you can use the save feature to make sure you survive.
Once completed it will be awarded to you both!

Simply finish stages 1-9, in local co-op, and the trophy is yours.
If you are doing this solo with 2 controllers (or 1 controller and reassigning the controller) you can let 1 player lose all their lives and not continue. The trophy will still pop when you finish the game with just one player.
You will also collect Win if this is the first time you have beaten the game.
Beat the final stage with another person on your 360.
