This is achieved by completing Floor 5 on the Kelvin Floor Heroic Assault.
Progress through the story until you unlock the Heroic Assault which can be accessed at the top of the map - it appears like a crime. Enter through the satellite building and choose the Kelvin incident.
These floors are high levels. I started Kelvin incident with equipment in the low 40s. However this instantly jumped right up when collecting the boxes and defeating the enemies.
Floor 5 requires you to fight Mr. Freeze in the Mecha and Harley Quinn. The battle progresses tougher as enemies begin to spawn as well. This is made much easier when done in multiplayer where you can help revive one another.
