This isn't nearly as hard as it sounds. It is important to note that getting hit three times counts as a death, but so does pressing to reset a level. The only way to reset your death counter is to pause and quit to the main menu, then select the level again.

To make it as efficient as possible, I highly recommend quitting to the main menu after each failed attempt. This is especially important in the later levels that are more difficult. You definitely don't want to finally complete a level after multiple attempts only to have to do it again because you couldn't be bothered to quit to the main menu to reset your deaths. So make it a point to do so after each failed attempt despite the hassle.

If you use this tactic, then simply completing all levels will unlock this achievement. Unfortunately the ingame counter and stats for each level seem to be bugged. Starting from level 12 onward, my completed levels no longer show my distance and deaths for that level. I'm not sure if this will be the case for everyone, but to be safe, never move on to the next level until you complete the current level with zero deaths, so you don't have to come back later and figure out which levels you still need to do.

As far as how to complete each level, I have included a link to a YouTube Playlist that shows all levels being completed. There are likely other ways to complete many levels, but this will get you through them all. The videos aren't guides though, so you'll have a couple failed attempts before each successful one, so just fast forward to the last attempt of each level to see how to do it.
