How to unlock the Is That All You've Got? achievement in Grand Theft Auto III – The Definitive Edition - Definitive Guide
In Grand Theft Auto 3 you are required to do the following to get 100% completion. You can also check stats for most of the requirements along with the percentage completed by clicking the pause menu and going down to the stats.
As the player progresses through the storyline, multiple gangs will become hostile to Claude or have their weaponry upgraded, making vehicle missions and Rampages extremely difficult. It is heavily advised to complete:
All Portland vehicle missions and Rampages before "The Pick-Up" and "Trial By Fire" (where the Triads become hostile after one is completed), "Sayonara Salvatore" (after which the Mafia become hostile and begin carrying Pump Shotguns), "Uzi Rider" (where the Diablos become hostile)
All Staunton Island vehicle missions and Rampages before "Kingdom Come" (where the Yardies become hostile), although the Colombian Cartel are hostile from the start of the game (but won't typically be encountered in open play until after "Last Requests")
All Shoreside Vale vehicle missions and Rampages before the final mission "The Exchange" (where the Colombian Cartel replace Pistols with Uzis and AK-47s).
Cheats disables achievements.

If you have beaten the missions by D-Ice on another save you will most likely get the “purple nines glitch”. This glitch will cause the purple nines to never spawn making the first mission impossible and the 100% impossible. The only way to fix is to delete all your saves files from cloud, local, etc. You will not run into this glitch if it’s your very first time playing the missions. This glitch was fixed with update 1.04 and you should no longer run into this issue.

The guide by Busterfrosty is pretty much perfect, but I just wanted to add something for those struggling with the Rampage missions. I found this tip on a YouTube video from GTA Series Videos.
You will need to get a one star wanted level close by a Rampage mission, by either bumping a cop car or running over someone in front of a cop. You will then need to get the cop to chase after your car while he is on foot to the Rampage icon. Next easily run over the cop (don't kill him) & then park your car on him. This will prevent the gangs from attacking or fighting back while you easily pick them off. Just stay kind of close to the cop to keep your 1 star active.
Alternatively (and even easier), if you have already completed all of the story missions, you can use the Rhino (tank). It is located in Phil's Army Surplus at the very north part of Staunton Island (The middle Island). Kills in the tank will count toward your Rampage goals except for on the missions where you must get head shots. If you are having trouble with the headshot Rampages, you should try the cop/one star method mentioned above.

MapGenie has an interactive map that I used to keep track of my progress and to plan the next step. A few items to keep in mind:
1) Some missions will close if you (spoiler) kill the mission giver. MapGenie lets you know which missions will close if not completed before hand.
2) Rampages need to be completed to get 100%, but there is no way to track it from the status page. Track it by the amount of money you get as the reward. It's $5,000/rampage completed cumulative. For example, completing the 19th rampage will net you a $95,000 reward. Also, you get a $1,000,000 reward for completing the challenge.
3) Do the paramedic mission and the taxi mission early in the game, before you start any missions after Luigi's Girls. Once you start working the different main missions, rival gangs will start attacking you, making these missions almost impossible. Also, health is hard to find in the game and you do not heal when saving, like in future titles. Get paramedic done early and you will have health and adrenaline spawn at the hideout.
4) when opening a new island, I go in this order:
Get all hidden packages.

Police Enforcer- can be stolen from the Staunton Island Police station. You need another police car to open the gates.
FBI car- spawns during a 5-star wanted level
Rhino- spawns during a 6-star wanted level. Also spawns at Phil's after completing the final mission.
7) Getting the FBI car and the Rhino can be a little harder. Wait until you open Shoreside Vale, and go to the safe house. Heal up, armor up, safe, and reload. This will reset the armor spawn. Start attacking the pedestrians and blowing up police cars. Blow up the helicopters, but make sure you are out in the open, so you don't blow yourself up. Hide in the garage, with the closed door as cover. Shoot from there to increase you wanted level to 6 stars. Once the Rhino tank spawns, wait for the soldiers to get out. Shoot them quick, get in the tank, and drive to the garage. Save and reload. Repeat until you get a 5-star wanted level for the FBI car. You may get lucky and get the FBI car and the Rhino at the same time.
8) You will earn a lot of money. I finished with about $8 million. You only really need about $600,000 to start missions, with $500,000 needed to open the last mission. Don't sweat if you need to stop and buy armor and weapons.

This method works to get 100% even if you missed a mission or two.
Repeat Paramedic Level 12, each time you complete it, you gain 0.68% progression towards 100%. the achievement unlocks once you reach 100% regardless of whether you've completed all the missions or not. I personally did not do _any_ rampages and missed one Mission on the mission list, that put me at just over 98%, i did Level 12 Paramedic twice and i got 100% and my achievement unlocked.
This is a very old glitch that existed pre definitive edition and i can confirm _right now_ it still works.
