If you flew under all 14 bridges and the achievement didn't pop out then it's possible you didn't fly right under the first bridge (from the sea) in Alderney’s industrial park. It counts if you flew under it right from smokestack. First I flew the bridge under the water and it didn't count.

To obtain this achievement you need to fly under all 15 main bridges in liberty city. So you must go through all of liberty city bridges and when go under the right bridge on your top left corner there will be a text saying "14 more under the radar bridges or something like that. Good Luck! P.S I do have this achievement
you can spawn an annihilator 359-555-0100, keep activating health cheat 482-555-0100 to prevent the chopper from being shot down. this way you can get the achievement early in the game before you unlock the other islands. use the map as a guide

Every time you fly underneath a bridge, you'll see trophy icons appearing on your mini-map. Press start to see the whole map, with trophy icons at the bridges you didn't fly under yet.
You can get to a helicopter as soon as you get to the central island (Algonquin). There's a helicopter tour business on the south side. Also, once you get to the big island, if you have done all Brucie's missions, shortly after getting to Algonquin he'll phone you to ask you on a helicopter ride, you can just do it during that 'mission'.
Another tip: do this while you can't yet officially get to the last island. That way, when you get to those bridges, you'll get a six star wanted level and you can go for the One Man Army achievement. Staying alive for six minutes while you're high up in the air is a piece of cake!

This achievement can be unlocked in either the main story mode or in The Lost and Damned. Go to a heli-pad and hijack a helicopter. You might get a one star wanted level, but it is easily lost. Now, look around for a bridge, shouldn't be too hard. Find one, and fly under it. It you've already flown here, nothing will happen, so find another. If it's a new bridge a notification will appear telling you how many more you need. Now, quickly press start and go to your world map. All around there should be little icons labeled trophy, and appear as such in the icon. Set a course for the nearest one and fly under. Rinse and repeat. There are a few bridges that extremely low to the ground and dangerously close to the water that you might fly by and think, theres no way they want me to fly under that. Well, sorry to say, they do.
