Looks like, my solution on the x360 still works, so I'll share it again
After completing:
You will unlock "San Fierro" and also you will respawn here, in Angel Pine:
In this exactly point, is the ambulance:
Is very easy to complete the paramedic missions at this point, because you only have to be around inside of that small city, doing so, you will save around 30 minutes or even more than trying this on a main city.
Mission Failure
- The Ambulance Falls Into Water.
- The Ambulance Blows Up.
- The Ambulance Flips Onto It's Roof.
- You Kill A Passenger. (Running them over or crashing the Ambulance too much)
- You Are Arrested.
- You Exit The Vehicle.
- You Push .
- Your Time Runs Out.
Completion
You will need to complete Level 12 of the Paramedic Missions in order to finish this Sub-Mission. Level 12 must be done consecutively, and altogether you will save 78 people, however this may increase in your stats if you fail or cancel the mission. The average time for completion of the Paramedic Missions ranges from about 30 minutes to 1 hour and 30 minutes, depending on location, however I'd advise Angel Pine if you can get there.
Rewards
-Upon completion of Level 12 your health bar will be increased to it's maximum, resulting in your health raising by double or more. This of course helps greatly on missions, or random gunfights with enemy gangs or the cops.
-And of course the achievement.

The ambulance wasn’t there for me at first. Had to save and re load the game to spawn it. Got it first go at this location though (got to wave 7 for the Glen Park hospital then game crashed)

I just did this one with your method thank you :) took about 25-30 mins but like you said you can do it at the very start of the game I have not even completed all the intro missions yet

You need to do all 12 levels of the paramedic missions in one go. For each level you have to pick up a corresponding number of patients (level 1 = 1 patient, level 2 = 2 patients, etc) and take them to a hospital. Your ambulance can only hold 3 people however and you can only return to the hospital when you are full or when you've got the last of the patients on board.
This can be done anywhere, however it's easier in smaller areas with less traffic. Once you leave Los Santos, you'll end up in Angel Pine. This is the ideal location to complete this as it's a very small area that you won't need to leave, so routing is simple.
The hospital is marked on the map and the roads within this map are the only ones you'll need to pick people up from.
