Also what I did, this is by far the easiest place to do it as all the passengers spawn in the small town, takes maybe 10-15 mins

Want to get this early after the first mission? No problem! Near your Grove Street safehouse, there's a hospital right nearby with a parked ambulance.
You'll want to take it all the way up to Fern Ridge located below:
Start the mission by clicking in , and pick up the nearby passengers, making sure to slow your movement upon reaching them. Make sure there's enough space behind you for them to get into the back.
Paramedic is more about keeping cool under pressure than being a full-on drag race. You get plenty of time in the much later rounds making return trips less of a problem.
There are 12 rounds in total which must be completed in one sitting.
The average time to complete it in this area will take 35 minutes.
You cannot leave the ambulance or kill a passenger as these are fail states.
Completing them all will give you a permanent health boost, which is really useful this early, and more so later on! Here are some points you should keep in mind when you're making your way around.
* AI drivers will always come to a halt when you're picking up passengers, however you should be careful about your surroundings as it is possible for the passenger pathing to get caught in a mess if there's no way for them to get to your ambulance. Case in point, having a vehicle block your back doors.
* The ambulance is prone to tipping over if you spin out. Going over small hills and bumps is never a viable option even if you want to try taking detours/shortcuts. You should always follow the road and mind incoming cars to avoid potentially killing your passengers and damaging your ambulance. It can only take so much punishment until it catches fire.
* Try to keep your thumb away from the to control the camera. Yes, it's a state of habit, but it can also potentially ruin the whole mission if you click it in. Take your corners carefully and let the camera swivel back to your rear.
* Plan your routes in advance. Check the map at the start of each round, look at where your passengers are spread out and keep your waypoint at turns. You don't always need to pick up 3 passengers. If there are 2 in one town, and four in another, get them in 3 groups of two. You'll learn pretty fast how crucial it is to get them back in even numbers.
* When driving up hills, it may be difficult to see vehicles in front of you. Always take it slow in this case as once again, time isn't an issue, and you'll more than likely collide with someone.
* Rain and stormy weather will impact your driving. This is something that occurs randomly during the in-game time, but this will make your trips slightly more tedious.
* Watch out for cops. Always.

This is most easily done in Angel Pine due to the small town size. Kill someone or find the hospital and steal the ambulance, then complete the mission as normal. Should be relatively easy as long as you don't kill any of the patients. Completing this mission set will give you 150% health.
