Before you can unlock this achievement you will need to have unlocked all the story characters, the vast majority are unlocked via the story itself but there are a few from random encounters. There is a list here of the characters and it shows which mission unlocks them (and of course details that mission) http://gta.wikia.com/wiki/Director_Mode I only did the story and Kifflom and I was only missing 2, Beverley and Mrs. Thornhill.
Dylan XBA also notes that Dr Friedlander is also missable but https://www.trueachievements.com/chasingghossts.htm confirms you can visit at any time to unlock him

Once you have them all unlocked, whilst story mode is loaded up press

and scroll right using

to the 'Rockstar Editor' menu and press

to enter. Choose 'Director Mode' from here and accept the launch.

Click actors and pick any of the actors in the Story characters menu.

Once the mode loads the achievement will pop, BLAZE VII notes that this one might have a little delay before it pops.

Assuming everything went well with your unlocks, you can also unlock the Cris Formage achievement if selecting him when on Director Mode:

Every character is unlocked apart from Cris from age ive done the kifflom mission and completed it but he still locked and it wont let me redo the mission 😠😠

I don't remember if you can visit the doctor again, Dylan XBA but if you could that might do it? I will add the Dr to the list of missables, I didn't realise id done that.

I don't know what to say Brev2034, if hes not showing and you cant redo then id hate to say it but you might have to restart :/

Be interesting to see if characters carry over saves since then you wouldn't have to redo much.

I think I have figured out why hes not unlocked, I did the kifflom mission got the achievement and then had to restart as my save got corrupt when I restarted I don't think I did it again as I already had the achievement gunna try it again

No, not necessarily. I just happened to discover the shrink while scrolling through locations while I was in the taxi. You could take a taxi or drive there.

I have unlocked ALL characters. Including Dr Friedman who was missing in the list. I got the doctor, killed him. Have 27/27 story actors and it's not unlocking. Does it matter which actor you take? Or is it glitched by now?

I have seen it. Still used to the Xbox360 days where achievements unlocked, even if you were offline.... Did it just now, achievement popped....

So I don't have Cris Formage unlocked but I have 100% game completion and have the kifflom achievement. I have taken the test again but nothing happens. Any help?

I really don't know, unless unlocked characters carry over saves so you could start a new save to get cris and he updates with your other unlocks.

I didnt have only Dr. I just went to him and... that's all ;) Cut scene, chase, I killed him and he unlocked.

Don't forget you need to also enter director mode as one of the story characters for this.

Achievement is bugged for me, unlocked them all, and entered director mode as one of them, but still no Chievo.

It looks like Lamar Down is the latest story mission you need to complete (alongside various Strangers and Freaks missions that unlock characters).

I can confirm that the dr is not missable! I get him before the jewerly heist. What a did was just see my messages there i saw a message from the dr saying that we need to do a session from the phone, then i just call him and ACCEPTED HIS ADVICE, then the call ended and i got the character!

Just a heads-up for everyone that had all characters already (like me). There was a slight delay for unlocking this one. So just give it at least a minute or so to make the game recognize it. Did the Cris Formage which unlocked straight away but not this one so I walked around for a minute.

If you had already finished the Kifflom! story, do not worry. I managed to get the Missions all over again (for some reason). Maybe they updated the missions in the even you have already completed them.

Go to the website on your phone and take the Test again. Thats how it showed up for me.

I can’t get the therapist Dr. Friedlander. The wiki says I needed to play Chaos and that the mission went away after the jewel heist so this achievement appears to be missable

I had 100% the entire game and just completed DLC's as they came along. I noticed that I did not have the blue ? for this type of mission. I ended up going to the Kifflom website on the cellphone and just took the test again and received an email from them shortly after and the blue ? appeared. That is how it worked for me.
