The trophy is not difficult, there is enough time. It is more important to save the car. You cannot leave the ambulance, the progress will be immediately reset. The car is not repaired at repair points. The ambulance can only accommodate THREE patients. There will be more and more patients at each level, so it is better to plan the route through the map to choose the most profitable one.

The whole task will take you 1 (+-) hour of real time. The main enemy here is not time, but AI that is trying to destroy your car. The ambulance has a siren, when it is turned on, cars drive away in the opposite direction from which you are, BUT only in the upper lane, cars only stop in the oncoming lane. I did it on the second island, mostly "sick" ones appear near the stadium, in Little Haiti and at the military base near the airport (there is a shortcut to the airport near the Cuban bar, I used it all the time).
