This is missable!
As you're going through the story of this DLC, you'll get a scene where a golem spawns and attacks. Kill it. Now you need to kill 3 more before you find out who is sending them after you. If you find out before killing them, you'll miss your chance for this achievement.
They are as follows:
1. The one you killed after the cut scene
2. Near witch hill, just south of the burned out house
3. Near the trees south of the gypsy baron's camp
4. South of the house at the quarry

Need to kill 4 golems sent to ambush you. First one you encounter during quest. Location of other golems: near quarry, near fishing pond, near burnt house. Missable if you progress too far in quest line!

4 ambushes are found after getting the missing leg for the Ghost in the Misty Quagmire and they are found in the following order:

Missable trophy. The first ambush will appear after you have been in the foggy swamps, or rather, after returning from them with the marquis, a golem will be waiting for you, so this is the first one. There are 4 ambushes/golems in total. The second one will be just above our vineyard, it’s not far from the nice house. The third one will be above the Talking Skull tavern (it’s from the DLC) or to the right of the fishing lake in the village. The fourth and last one will be in the quarry, if you move with a stone, then when you leave the house in the quarry you will stumble upon it, killing it will get the achievement.
