I played the game through completely on Guitar, and I received this achievement. I did not touch a microphone at all during the process. I can't sing, I don't even bother with any of the vocal achievements unless I have someone else do it for me.
However, I received this achievement in Xbox Live Band Quickplay. The setup I had when the achievement unlocked:
I finished all songs that I could with guitar previously, in solo Career, and I had 5 stars on them all.
I was playing with someone over LIVE who was on Mic and Bass when this achievement unlocked.
I selected "Song of the Century", and he 5-Starred it.
AFTERWARDS we played a song that required all instruments, and after the song both achievements unlocked for me. [3star all songs and 5star all songs].
So, I assume if you play Song of the Century first, then play another song and 5-star it, you SHOULD get the achievements.
Unless it was just a glitch, I'd advise you to try this if you, like me, refuse to pick up a microphone.

This achievement can be attained across different instruments instead of all the same, it requires any difficulty, and the stars from career and quickplay both count towards it!

Thanks! This was the information I was looking for! So I can combine instruments. Does not need to be 5 star on just one. And also it cant really be done since one song only has vocals for example. So I don't need to do them all on vocal then.

This one is very easy. Once you 5 star every song in the game you will get the trophy, You can play the songs on any difficulty. To ensure you get the 5 stars, play on either easy or medium.
Earn a rating of five stars on each of the 44 songs on the game. This can be done on any difficulty with any combination of instrument in Career and/or Quickplay. I would suggest doing this on Bass because you need to earn at least three stars on every song on Bass to obtain “In Good Health and Good Time”. You can check your progress for this and all other achievements in the “Achievements” menu in game.
NOTE: “Song of the Century” can only be played on Vocals. “Good Riddance (Time of Your Life)” does not have a Drums chart.
