The Cult of Bysmiel can be found in the Conclave of the Three. This area can be reached following the completion of Act 1. A new NPC will appear in Devil's Crossing through the southern gate. Speak to him and he will open a green portal that takes you to this new zone. There's a quest early on in this area that makes you choose between one of three Cults to help. Whichever you choose to help will give you a set a of quests you can complete to progress that particular story.
Rep is earned through quests, bounties, and killing monsters. Revered is the last reputation you can reach with a faction. It takes 25,000 rep per faction to reach Revered.
A good way to cut down the grind is to choose a different Cult in each difficulty playthrough. That way you will have access to each questline and it will give you more rep per faction overall.
When you reach Honored rep status you can buy the writ (piece of paper item) for 50% rep gain.
