Burrwitch Village is a world area in Act 1. It has a Riftgate and connects to Burrwitch Outskirts, Burrwitch Estates, Abandoned Waterfront and Mud Row. In order to get this achievement you must secure the Burrwitch Village riftgate in Veteran Mode.
Veteran mode can be toggled on from your options menu, or it can be selected at the start of your journey. If you don't want to play Veteran Mode from the start, you can toggle it on just before the Riftgate. Or you can wait until your Elite playthrough where you will get it anyway. The game counts "Elite" difficulty as having Veteran Mode toggled on. So when you secure the riftgate on Elite difficulty the achievement will unlock. That's how I got it.
