This achievement can either be achieved fairly quickly or take a relatively large amount of time depending on RNG and the clear speed of your build.
You will need to have all five of the Shattered Realm shrine buffs active at the same time, if one of these buffs wears off it will void the achievement and you will need to refresh your missing buffs.
The strategy I used here was to keep pushing shards and refreshing the buffs whenever I came across a shrine. I was able to get four shrine buffs through shards 1-60, however I did not see the fifth shrine until shard 64. This can be done in normal mode as well to make the process quicker.
