This achievement is missable and occurs during Year One of the game.
The balloon twister can be found on the streets of El Marrow, outside of the Department of Death. In order to get through the game you'll need to interact with him and get balloons from him - but if you talk to him and progress through the conversation, the dialogue option "BANG!" will occur. Selecting this will scare the clown and make him pop the balloon he's working on.
You'll get the achievement at the end of the conversation.
