New Game Plus only
To start a New Game Plus,
- open a file where you have beaten the final boss ("Inevitable" achievement)
- go to any save point
- at the save point, select 'Reality Shift'. This replaces your playthrough 1 file with a NG+1 file.

I hope it;s possible (or will be with another update) to do a reality shift again and go to NG+2. On Xbox the achievement didn't unlock when the requirements were met (it did on Steam, had problems with how achievement progress showed up on my profile per game e.g. 0/1000, it got fixed). Now I need 2 more breathearts to get another weapon to +9.
