To rescue MOM, the first thing you'll need to do is collect all 9 ship parts scattered around the planet. Each one is marked with a big box outline on your map that is impossible to miss when you scroll past it. Once you find a ship part, you have to climb on top of it, then jump and grab POD's magnet and pull it down onto the ship part so he can take it up to the moon.
Three of the ship parts are found on high islands that you need to reach by flowering a Starplant. Refer to the "Star Planter!" achievement solution for more information on flowering Starplants. Each of the first three Starplants will take you to an island with a ship part.
The remaining six ship parts are scattered on islands around the planet. After collecting one, pull up your map, find the nearest one, set a way point with and make your way over to it to collect it. While doing this, as noted in the Roadmap, keep on the lookout for abilities, telerouters, and crystals while making your way from ship part to ship part. The more stuff you collect in between, the easier your life will be.
Once you've collected all nine ship parts, you now have to go to the moon to reassemble the ship. Getting to the moon requires you to ascend a bunch of floating platforms using floraforms and your jetpack to reach the base of the fourth and final Starplant. Thankfully, this Starplant doesn't need to be flowered, you just have to climb to the top of it, grab the bulb, and press to automatically grow it right to the moon.
On the moon, MOM will be laying there assembled. Simply walk over to it and a cutscene will trigger with the ship assembled. Go inside the ship and up to the Control panel and pull both switches to boot MOM back up. This beats the game and unlocks this achievement.
Note that you can continue to free roam after beating the game.

Rescuing MOM is your main goal in this game. When the ship crashed on the planetoid's moon, MOM parts were thrown into various locations. When you approach a part, POD will ask you to grab the magnet and will lift it of to the moon. These are the part's icon:
Grab all of them, and then head to the moon. Once there, the parts will merge together, and the trophy will unlock.
Some of them are easy to reach, while others require a bit of effort, or even some abilities you need to unlock by collecting ability pods/crystals.
