In his first form you have to be running all the time while he is invisible. If you stand there's not enough time to dodge once he appears, escpecially from above.
In the final fight it comes down to knowing his poses before he attacks. If you know them it gets a lot easier. When he's breathing fire you can stand directly underneath him and dodge the attack if you roll at the right time. This way you can start hitting him earlier.

I defeated him, but no achievement. Also, it automatically started a new game for me so I couldn't revisit anywhere. Have I glitched my save?

i haven't heard of the boss fight beeing glitched, but that doesn't sound good. as far as i know you can go back anywhere in the world and fight him as often as you like after beating him. did it override your save with the new game?
