How to unlock the Combo Nerd achievement in Guacamelee! Super Turbo Championship Edition - Definitive Guide
In El Inferno you can get literally a never ending combo. In Challenge 3, enemies can only take damage when you pull the lever and kill them with the spikes.
Take out the two flying chupacabras and one skeleton. Now keep hitting the skeleton thats left until you hit 125 and 200.

A perfect fool proof solution that requires no skill at all. Just right for me. :)

This how I got "Combo Nerd" and "That was INTENSE"
When you get the Objective "Find your way through the Canal to the Desert" you will come across a boat after a little bit of running around.
Get on the boat.
The boat will automatically start advancing and a crap ton of enemies will spawn. Use you INTENSO Meter and you should dominate them all.
I got up to around 150 and was hit a little before it ended. It is probably possible to get up to 200 here. I don't know.
EDIT: user spockroxfaces added this, "it is possible to get over 200 hits here. If you don't get it then quit out and reload and you'll be right before the boat. Your method is correct I would just add the fact of staying around the center of the boat because of the fish that jump out of the water. Make sure to use the LT dodge roll liberally and you'll get it within a couple tries. Hope this helps."

In the level Temple Of War, starting from the teleport stone, make your way up and to the right as shown from 0:11-1:17. This was my first time through so I apologize for the missed jumps.
I did not realize I was going to fight so many enemies, which is why I turned the intenso off at 2:02. Then, when I realized how many enemies were coming, I reactivated it at 2:05 and unlocked both achievements for the 125 hit combo and 20 consecutive kills in one intenso sequence.
