It's 5 days late now. But if you connect a second controller and it does not show in game, it might be, because the second controller is not associated with a different gamertag? Have two gamertags on your console and each controller be logged into a specific gamertag. That should do it.

With one more controller you should also avoid cloud streaming to make it register/appear. Doesn't happen when downloaded. Then just sign in as guest and you'll get the suffix "(1)" after your usual gamertag for the 2nd controller.

A Perfect means you did not take any damage during the match, not even chip damage. You either need to win in an overwhelming manner or you need to do this in VS mode with a second controller. You can block attacks as long as all of your blocks are Faultless Defense.
