This achievement is quite easy to get by yourself or with a buddy. Just start a new song on expert, strum until it fails, and when it asks to retry song, rinse and repeat 4 more times and the cheevo locks. Just plug in a 2nd controller, etc... and make sure it's a regular co-op song.

This achievement has a suprisingly high ratio for how easy it is!
First off, even though it's a co-op achievement you can get this on your own provided you have 2 controllers, set a local co-op game up and pick a song, make sure you're playing on expert and when the song starts up just strum wildly until you fail the song, upon failing you'll be asked if you want to retry, retry the song and strum out again, rinse and repeat and you'll have this achievement in no time!

The easiest way to get this achievement is to set up a local co-op Expert career. Choose the first song and don't play a note. You will fail once you are 20% into the song as you cannot fail before that time. Repeat 4 more times and the achievement will unlock.
This can also be done online if you set up a 5 song setlist in co-op. If you fail the first song after 95% you can get Fallen Angels then fail the next 4 by strumming before the song starts as on LIVE you don't have to wait for the 20% point of the song to fail. The achievement will unlock after the fifth song has been failed.
