How to unlock the Medium Tour Champ achievement in Guitar Hero II - Definitive Guide
Difficulty achievements do NOT stack.
Note: Although it is possible to use a standard controller to play, it is suggested that you purchase a Guitar Hero controller. While using the standard controller, however, you will use the LT, LB, RT, RB, and A buttons to play the colored notes.
Unlocking this achievement:
Start a new career from the main menu on Medium, name your band, etc. You will need to beat all 48 songs in this game on the Medium difficulty. You will also be using the first 4 fret buttons (Green, Red, Yellow, Blue), but not the final Orange button.
Medium difficulty is fairly simple, but is much harder than the Easy difficulty because it introduces 2-note chords, occasional hammer-ons and pull-offs, and the 4th blue fret button on the guitar controller. If you are a beginner or unsure of the game's features you may want to try the tutorials before starting your career to learn about Star Power and the other features (Training - Tutorials).

You aint gotta beat all 48, just 4 each time and the last song wants all 5. So you can do 41/48

Finish Career Mode on Medium Difficulty. Four frets here.
