The earliest you can earn this achievement is at level 15. At 15 you unlock safety net which is the fifth power. You do need to use each power at least once in a song just equipping them isn't enough.
Any combination of the following will unlock achievement:
Clear Highway
Dial Down
Dial Up
Double Multiplier
Safety Net
Invincibility
Score Chaser

Hero Powers are power ups in GHTV that temporarily alter the gameplay. They can be obtained by hitting all of the hero power notes (those of a blue colour with a lightning bolt in the centre) in a row. Then, to activate it, either press the Hero Power button on your guitar or tilt the neck of the guitar upwards. Different Hero Powers are unlocked as you level up and they alter the gameplay in different ways such as temporarily removing the notes in the highway or lowering the difficulty for a few seconds. The fifth is unlocked at level 15. Before you start each song you are presented with a list of all the Hero Powers that you own. Play five different songs using a different Hero Power each time and so long as you activate it at least once per song, then this trophy will unlock.
