Ok first of all let’s say this, you will NOT unlock the achievements for completing Easy, Normal, and Hard if you beat the game on Insane, you are going to have to play the game four times, once on each difficulty, it is recommended that you start at easy and progress towards Insane because then you will have a good understanding about the game. Here are some helpful tips that should help you get through Insane Mode.

• Don’t forget to look around corners, this helps to pick off enemies without being fully exposed, to do this just press or .
• Never rush into an area, stay back and pick off enemies with a sniper or a rifle, you WILL die if you try to be a hero.
• Always ensure you have a full whiskey bottle and ammo before starting a new mission, there's nothing worse than running out of ammo in the middle of a fight.
• Complete as many side missions as possible so that your stats increase as your progress through the game.
• Buy a Pickaxe and mine some gold, then go and buy as many upgrades as you can afford, they do help.

Good luck and if you manage to complete the game on Insane Mode, Congratulations.

I strongly advise just using this cheat method just to save a LOT of time and frustration, as Insane difficulty is incredibly difficult and frustrating. Here are the instructions on how to do the cheat:

1. Beat the game on Easy and keep your save file (you’ll need this file to cheese the side missions using this cheat as well).
2. Start a new game on the difficulty you want to earn the achievement for (for the sake of this achievement, pick Insane).
3. Turn slightly right and walk to the river bed and go in between the two small rocks, now turn around to face Ned.
4. Hold ++++. You’ll here a peculiar sound effect. Keep holding the buttons until the sound stops.
5. Pause the game and go into the Options menu. You should see a new tab called “Gun”. If you don’t, repeat step 4.
6. Click the “Gun” tab, then select Missions > Main > Magruder’s Mine.
7. When the mission loads, pause the game again and go into the menu again, but this time hit Checkpoints. Scroll down and select “Kill Magruder Pt 2”.
8. When the mission loads, pause the menu again and select the option Toggle Help, this will give you all of your guns (this is why you must beat the game at least once on easy, to unlock all of the guns).
9. Turn around and look at the small rocky structure. Take cover behind the taller rock on the left and select the sniper rifle by holding and .
10. If you did it right, he will not be able to hit you. Hold again to poke your head out. Press to zoom in on the scope. Shoot Magruder in the head with the sniper rifle.
11. When he throws the dynamite. Press and shoot the dynamite out of the air. Repeat 4x to kill Magruder and earn this achievement.
12. Repeat steps 1-11 on Hard difficulty and Normal difficulty for their respective achievements. This should take you about 10-15 minutes or so all together.

IMPORTANT: There is also a way to do this to unlock each of the various side mission achievements as well, and it DOES count towards the 100% total needed to unlock the last weapon for the “Every Weapon Collected” achievement. To do the trick, here are the steps:

1. Do steps 1-5 above if you haven’t already (I’m going to assume you have).
2. Load up your easy save file (this is why you must keep it, as stated in step 1).
3. You should see the “Gun” tab in the Options section of the menu.
4. I’ll use Poker as an example. Select Missions > Side > Side: Texas Hold ‘Em > Scroll down to the last one.
5. All you have to do is complete the last mission of that type to unlock the achievement. Meaning, for Poker, instead of having to beat all 6 rounds of the tournament, you can simply load up the last one and beat that. This will unlock “Card Shark” and add a nice chunk to your completion %.
6. Repeat steps 3-4, but do it with another type of side mission. It can be Hunting, Peace Keeper, Bounty Hunter missions, Pony Express or Horse Challenges/Ranch Hand activities. Simply select the last one of each type and complete it for that respective achievement. The only one I had to do every mission for was Pony Express. Using this method for the last one will earn you the achievement, but it will not add to your game completion %.

Note: If you turn your Xbox off, the menu will disappear. Don't panic though, you can make it reappear by exiting the game and then performing steps 2-5.
