For me, it unlocked on PC shortly into my third playthrough, on Hard.
I still had:
- Hard completion
- 500 demo attacks
- All grave stats
- All grave skills
- All lab upgrades
- All Bunji upgrades
left to do when it unlocked.

Same for me, this achievement unlocked without me finishing the game in hard mode.
I think this bug is because of the free DLC, the game has 49 achievements with 3 achievements in the DLC, so a total of 52.
This All Achievements Unlock will unlock when you have 48 achievements, it does not matter if those achievements are coming from the DLC or main game so if you grab 1 or 2 achievements from the DLC, you can omit 1 or 2 achievements from the main game and unlock this final achievement “All Achievements Unlock”.
