How to unlock the Hold the Onions achievement in Hades - Definitive Guide
After buying enough things from the house contractor, you will be able to purchase Erebus gates for the underworld for 5 diamonds. These spawn randomly like the chaos portals, indicated by the flaming portal in the floor.
They are locked by heat ratings, with a Tartarus Erebus gate requiring 5 heat, the asphodel one requiring 10, and 15 heat for elysium.
These rooms will require you to fight a few enemies without taking any damage for a reward, but if you take damage you will not receive that rooms rewards. Obviously the Tartarus ones are gonna be easiest just by nature of only being 5 heat. Once you successfully complete one and earn the reward the achievement is yours.

I've done this 3 times now and no achievement pop. I wasnt hit, got the rewards , and nothing. Does anyone know if this is glitched?

Achievements are glitchy in this game because quick resume. I’ve seen some people close out and restart the game to get the achievements

Requirements: This cannot be done until you've completed at least 1 full clear and have unlocked the Pact of Punishment. Also, this trophy requires the purchase of the "Gateways, Erebus" job from the House Contractor for 5 Diamonds (requires: final boss completion, Contractor's Desk Deluxe purchase).
Once you've beaten the last boss once and unlocked the Pact of Punishment (see Champion of Elysium ), you'll unlock Heat levels which increase the difficulty of the game by adding conditions. Once you've purchased the Erebus Gateway Work Order, apply whatever mix of conditions you like until you reach a level of 5 Heat and begin your run. Similar to Chaos Gates, eventually you'll run into a chamber that has an Erebus gate. It looks very similar to Chaos Gates as a circular hole gate in the ground, but has chamber rewards and a Heat icon. However, while it may look similar, its function is far from it. Erebus Gates are like small challenge rooms in which you must clear the chamber without taking any damage. If you do, you fail, and you'll return to the top without the reward (and without this trophy). If you have trouble with this, I recommend using the Chaos Shield as your weapon, as you'll be able to hold it up and block any damage with it.
Note: Erebus Gates require different Heat levels to access depending on which area you're in. In Tartarus, Erebus Gates will need 5 Heat; in Asphodel, Erebus Gates will need 10 Heat; and in Elysium, Erebus Gates will need 15 Heat to unlock. No Erebus Gates appear after Elysium.
