How to unlock the Escaped Asphodel achievement in Hades - Definitive Guide
You will get this after you clear the 2nd level (the lava/magma level) and defeat the boss and move onto the 3rd level. Should come naturally as you play and remember to upgrade at the mirror to make things easier as you go.

I ended up clearing Asphodel a couple of times and the achievement never popped up. What should I do?

Quick resume causes achievements not to pop, but if you force close the game and reopen it they should all unlock.

Story-related and cannot be missed.
Asphodel is the second biome in the game and is made up of 10 Chambers that you must clear. (See Escaped Tartarus for more about chambers & rewards.) The final Chamber is the boss fight, the Bone Hydra (AKA Lennie!) While you'll always face Lennie, after you've beaten him a total of 3 times, he introduces a few different forms of himself and which one you'll get is random. That said, they're all mostly the same though, with just one or two different attacks. Lennie is a fairly straight-forward boss fight and is arguably the easiest in the game.

Lennie, the Bone Hydra:
Lennie has 2 Phases and they alternate. For the first 33%, he's in Phase 1, wherein he attacks you with volleys, lunges, and by slamming the ground. Once you've taken off 1/3rd of his health, he'll switch to Phase 2, wherein he retreats and becomes invulnerable. At this point, 3 lesser copies or "Support Heads" will spawn at each edge of the arena (far west, south, and far east). These new heads can also lunge, but also have 1 extra ability that Lennie does not. This extra ability is random and changes each time you fight them. Once you've dispatched all 3, the invulnerable barrier dissipates and he returns to Phase 1. Rinse and repeat. Once you've taken off 2/3rds of his health, he'll switch back to Phase 2, but this time he spawn 6 Support Heads rather than just 3, all of whom have the same attacks as the previous set. Finally, once all 6 are dead, he returns to his Phase 1 setting, and now all you've got to do is finish off the last 1/3 of his health and he'll finally die, rewarding you with a Diamond for your first kill (or Darkness for all future kills).
Note: Many forms of Lennie or his clone Support Heads can shoot out eggs. These eggs must be destroyed within 5 seconds, otherwise they will crack open and spawn a Bloodless (the normal humanoid enemy type of Asphodel). The Support Heads can also shoot out these eggs, and each new Phase increases the number of eggs each time from 1 to 2 to a max of 3. If you're not conscious of these, you can easily become overwhelmed. The best idea is to use your AOE Boons to their best. For example, Dionysus' Festive Fog cast, Demeter's Crystal Beam cast, or Ares' Blade Rift cast are all great skills that will help damage both the bosses and the eggs.
