How to unlock the One for the Ages achievement in Hades - Definitive Guide
This will be one of the last achievements you unlock, it requires that you complete the story, get the credits screen, and raise your relationships with the Olympian Gods. This will require a lot of runs and a lot of nectar (enabling God Mode, leveling up your mirror and weapon aspect, and using zero heat level will make this a lot easier). Spoilers for the main story ahead.
To start the epilogue quest:
There are 9 Olympian Gods, max out your relationship with 6 of them in order to advance. Keep giving them nectar and talking to them until you reached the locked heart, then use these steps to unlock the heart.
NOTE: Because of the random nature of the game, the necessary dialog won't always trigger when you talk to someone. They might say something unrelated to the quest and you'll need to start a new run to talk to them again. It took me a long, long time to get Poseidon and Hermes to finally bring up the right dialog to unlock the heart.
I've ordered them very roughly from easiest to hardest:
Demeter - You only need to get her to 6 out of 7 hearts. Just keep giving her gifts.
Ares - He will eventually give you a quest for slaying 10,000 foes total. You can track this number with the Scrying Pool in the Bedchambers. Complete the quest and talk to him again.
Artemis - Keep giving her gifts and she will eventually talk a lot about Callisto. After she exhausts all her Callisto dialog, the final heart will unlock.
Athena - She must congratulate you 3 times after a successful run. Just keep defeating the final boss and talking to her.
Zeus - Wants to hear what the others think of him. Pick up other boons while already holding a Zeus boon to hear what they'll say.
Aphrodite - Finish the 3 romance storylines. Then keep talking to Aphrodite.
Dionysus - Max out your relationship with 6 other characters (they don't have to be gods) and then he will ask for 10 ambrosia. You'll probably already have maxed out Dusa, Meg, and Thanatos for their achievements.
Poseidon - Buy the Rod of Fishing from the House Contractor and keep catching fish. Catch at least 18 fish. After, in a single run, catch a fish and then talk to Poseidon.
Hermes - Level up the Lambent Plume keepsake to 3 stars and have it equipped while talking to Hermes.
After you've satisfied at least 6 of these relationships, the quest giver will eventually ask you to speak to all 9 Olympian Gods to give them an invite. This just involves talking to everybody again until eventually the quest is complete and you get a final epilogue cutscene.

To reach the epilogue, after you've completed the main story, you'll get a prophecy called "The Queen's Plan" from Persephone, which will require you to befriend all of the Olympians to forge a bond and max your relationship with 6 of the 9 Olympian Gods/Goddesses: Aphrodite, Ares, Artemis, Athena, Demeter, Dionysus, Hermes, Poseidon, and Zeus. To do this, you'll need to keep gifting them Nectar until their affection gauges max out. This will take a lot of Nectar, so be warned. Certain dialogue scenes are also required for you to see, but as long as you're talking to everyone each time you return home, this shouldn't be an issue and you don't need to worry about it or track it.
Once you've finally completed this quest, Persephone will give you one more task: to invite all of the Olympian Gods, as per your new prophecy, "Family Reunion". To complete this quest, you'll need to collect 1 Boon from each of the 9 Olympian Gods. This, of course, does not need to be done in one run; it can be done in as many as you need. (If, after a few runs, you're still missing any, use that God's Keepsake to force their Boon to appear.) When you've spoken to the last one, the next time you speak to Persephone will trigger the Epilogue and will give you this trophy!
