Chthonic Companion's are required through the same method as Keepsakes, but become available when your relationship with specific characters grow to a larger level and by gifting them Ambrosia instead of Nectar. When you give Nectar to a character, their relationship grows and on the first exchange you will receive their keepsake.
As you progress in your runs, make sure you talk to each character every chance you get and when you have nectar available, give it to them as a gift to boost their relationship status. Eventually in the codex, you'll see their status shows a heart with a Keyhole. If you have Ambrosia (which is mainly unlocked during runs on the Elysium boss fight and through fulfilling prophecies), you will be given the option to gift it to them. A unique dialogue will play indicating that the characters have grown extremely close and you will be rewarded with their Chthonic Companion.

-Thanatos
-Skelly
-Sisyphus
-Dusa
-Achilles
Companions themselves are like Summons. They each offer a different ability you can call upon during a run. You start out with only being able to use them once per run but with added upgrading at the Display Case in the Courtyard, you'll be able to use them multiple times throughout each run.

You might have to talk to him more. The option for giving Charon Ambroisia only popped up for me after I had some more Talks with him even though I had maxed out hearts with him.

Yep, I believe if the option isn't immediately available when you hit the locked heart, it may take a few more runs and times talking with the character. Eventually you will be given the option.

Once you've got your first Chthonic Companion, you can equip them from the same place, and in the same way, that you equip Keepsakes. Interact with the display case in the Courtyard and click on the Companion in question and this trophy will pop.
The easiest one, and probably one of the first you'll be able to do, is Skelly's, and you'll have to do most of it for the trophies either way.
SKELLY'S FAVOR QUEST & COMPANION:
First, to get his Favor quest, you'll need to have maxed out his Affinity Gauge at 6 Hearts and you'll also need to have unlocked any 4 weapon Aspects other than Zagreus Aspects. Once you've got that, the next time you speak to him, he'll give you his quest. It's very simple. Level up the Stygius Blade's Zagreus Aspect to Level 5 (max), and then kill Skelly with it. Once you do, the next time you return to the Courtyard after your run, you'll be able to give him Ambrosia. Give him 1 Ambrosia and he'll give you his Chthonic Companion called "Companion Rib".
