How to unlock the Thorn of Thanatos achievement in Hades - Definitive Guide
To unlock this achievement, you must first acquire the pierced butterfly keepsake. You get it by giving nectar to Thanatos, who you'll run into pretty early on in the game.
Once you have the keepsake, you must get up to a 30% damage boost during a single escape attempt. You make the percentage go higher by completing encounters without taking any damage. It is very challenging to do this initially because you only get 1% per room at first. If you are struggling, my advice is to keep the butterfly on you until it levels up, preferably to its 3rd level which will give you a 2% bonus per room. Once you have a fully powered butterfly keepsake, you will need to complete 15 separate encounters without taking damage during a single run. Just be patient and play safe and you should be fine.
One final tip, I did this using the Chiron aspect of the bow and arrow so I could keep some distance between me and the action. However I'd say you could do this with anything that helps you maintain distance, such as the Rail, Spear, etc.

Something is a little off with this keepsake for me where it just arbitrarily stops getting stronger despite me clearing encounters while taking no damage. Stopped at 29% on my last run, and now it appears locked at 22% for my current run. Either it's glitchy, or there is some kind of hidden cap on the amount of encounters you are given to do this before it just stays locked.

I have reached 30% twice now so far, still no achievement pop...

Ranged weapons are your best friend for this trophy, even though you can go with any weapon you feel confident with. I also advice you to first level up the Pierced Butterfly to level 3 in order to obtain maximum bonus (%2) for each room.
If you are struggling with this, just turn off the auto-sync option for the game and manually upload your save file after every successful room. That way you can download and replay the last room whenever you screw things up.

TIP: I recommend waiting to do this until you've max ranked the Pierced Butterfly Keepsake to level 3. It will be much easier.

In case you aren't certain whether you've taken damage or not, on the left side of your screen underneath the Keepsake icon, there will be two circulating lights that indicate whether you are still eligible for the bonus +% for that Encounter or not. If the lights disappear, it means you've taken some damage in this room and thus the Encounter you're in will not count towards the 30%.
Once you reach +30% bonus damage (specifically from the Keepsake; extra damage from other sources will not count), the trophy should pop. You don't need to do anything with that like kill mobs with it, just reach that level.

Do not use the Pact of Punishment! Why make things harder for yourself unnecessarily?! Also, try to get this done before you reach the 4th biome, the Temple of Styx. This is because in the Temple, rooms are counted differently; only the final room in each tunnel/pathway counts as an Encounter for this Keepsake. This means you won't get any bonuses for all of the rooms you do leading up to the 4th and final room.
- The best weapons for this are: the Chaos Shield, the Heart-Seeking Bow, and the Adamant Rail, although the Shield is my personal favorite. If you're unsure or having troubles with this, the Shield is your best bet. You can block all attacks using the Shield, and blocked attacks count as no damage, so that will honestly drop the difficulty way down, if not outright trivializing this trophy. Alternatively, with the Bow or Rail, you can stay back and pick them off from afar. The Shield's Special also gives you ranged attacks. Either way, whichever you choose, feel free to play it safe; there's no timer for this, so take your time if you need to.
- Go for Boons that will protect you or help evasiveness, especially Athena's Deflect Boons, as they're definitely helpful in preventing damage. Deflect works very well with any weapon, but extra well with the Chaos Shield and the Heart-Seeking Bow. Especially with the shield! Chaos Shield + Attack/Special Deflect = Easy mode! It's even better if you use the Shield's Zeus Aspect (its 3rd Aspect; see Infernal Arms for more info) with the Special Deflect. For the Bow, try to get Deflect for your Special () rather than your Basic Attack, as the wide spread of arrows in your Special is really good because each arrow will Deflect. Hermes' "Greater Evasion" Boon is also a nice addition as a just-in-case, as it gives you a chance to dodge on hit, although that shouldn't be depended upon.
