Beat all 10 levels on Heroic solo or coop to unlock this. Difficulties do stack in this game also.

Bugged for me. I played the entire game on my first playthrough in Heroic, got all the achievements based around Heroic or higher difficulties...yet didn’t get this one at the end. I didn’t even get the one for Normal either.
Only thing I can think of is that it went offline at some point? I can’t think how though.
Oh well, I guess I’ll get it on my legendary playthrough.
Edit: Looking at chapter select - they’re all there, but AOTCR, 343GS and Keyes don’t have the shield with sword logo like the rest. Odd. I guess you can only make sure they’re there after each chapter as you play.

@TheCMHammond - Figured it out, if you're playing co-op with someone, after each level have them end the game, then get out of the lobby yourself and check your own progress, and then start up your highest completed level, and once in, get a checkpoint and then Save & Quit, it'll save your progress that way. Currently working co-op Legendary with a friend myself and this is how we've figured out how to do it.
Also, in Silent Cartographer, if Foehammer doesn't land right at the end for extraction, it can glitch the level incomplete and not unlock the next level in your progression however it will unlock the level after that for some reason, my friend and I ran into this problem until we went back into Silent Cartographer and made sure Foehammer was backed all the way up to the platform properly, that fixed the progression glitch.
