If you want to do this, make sure you do it from a save file where you have done ZERO of these objectives.
If you load from a save file which has done at least one of them, with a partner that has done 0, and you complete all of them, the achievement will not pop for you. (but it will for them).
Best to create a new save file specifically for co-op.

Confirmed. Have both players start a new save, speedrun through the campaign missions until you reach The Sequence, then you'll have access to all the FOBs on the map. Grab some banshees and fly to each of the 12 FOBs and capture them. No issues this way.

I just played with a group on a new save and we got the first 7 FOB's and then realised we had to progress story further to get access to the other 5.
Played up to the mission "The Spire" but some of us had to leave. My question is, do all 4 of us have to resume another time from the exact same mission for the achievement to work or can I play coop with someone else?

Hi, this isn't necessarily a guide, but more of a supplement: I was doing this with my co-op buddy, and I popped the acheivement, after liberating the last FOB, but he didn't. He and I both started the co-op session with one FOB unlocked (the same for us both, we were already sharing progress), but only one achievement popped.
Our solution was to go back to the mission Outpost Tremonious, and liberate one of the FOBs immediately after starting the mission. Once we got it done, he got his achievement. I'm not sure if this is going to be something universal, or if it's going to be something specific to our saves (prior to the FOBs, we'd done a 4 player LASO run), but it might help to know just in case.

One common way to get this is to start with a new/empty game slot, on Easy difficulty, with Bandana skull. Non-leaders of the fireteam can bring in any weapons, including tank gun. A fast way to get this cheevo is:
- Start a new/empty game (Easy difficulty, with Bandana skull)
- Go thru missions, ignoring all side quests & FOBs (except FOB Golf) until you complete Pelican Down misson
- Begin Sequence mission, kill the ~20 enemies at start of mission
- Press the button to cause the large bridge (to west) to get deployed ... but DO NOT complete the remainder of the Sequence mission (i.e. do not visit the four beacons)
- [optional] Grab the Banshees flying above the newly-deployed bridge, if possible. If not: simply run west across the bridge.
- Go to FOB Lima which is near the west end of the new bridge, up on hillside, and capture it
- Drive (or fly, if you have banshees) from FOB Lima west to FOB Hotel and capture it. FOB Hotel is important because it always has two Banshees.
- Get Banshees from FOB Hotel, and fly to all remaining FOBs and capture them. If Banshees are destroyed, "Fast Travel" back to FOB Hotel to get new ones.
