You will essentially need to do two playthroughs of the game with mostly gold medals, or three completed playthroughs without golds. You get more score added to your lifetime score by playing missions on different difficulties, replaying missions on a certain difficulty will not add much, if any points to your total.

Does single player and co-op stack? If not, can it be obtained via multiple run throughs in co-op? Thanks.
