On the first mission, you have to kill 100 grunts to reveal the skull.
When it's done do not kill the remaining ennemies in the base, and head back to the entrance where you found the last group of marines.
The skull will be on the wall opposite to the base door.
Note : On easy, they're only 97 of them (or so it seems) so you must do the mission on normal or higher.
The 1st mission skull is the easiest to collect, but you can do it in every mission.

I can confirm this EXCEPT that there is only 97 or 98 on normal too so im guessing you have to beat it on heroic or legendary.

I could never get 100 grunts on any difficulty on the 1st mission. So I tried the 2nd mission and got it 1st try.
On the 2nd mission all you have to do is destroy the methane refinery. All of the methane tanks and machinery are close to each other near the middle of the map. There are also a few methane tanks off to the right of the entrance to your base but I'm not sure if you have to destroy them or not. Once this objective is complete, the skull will appear at the back of your base.
I thought this was much easier than the 1st mission.
