How to unlock the Humble Servant achievement in Hand of Fate 2 - Definitive Guide
The Disciples Guise is awarded for fusing the 6 shards together, that's a separate achievement you need to complete first.
The Disciples Guise makes The Servant challenge more difficult by reducing your starting Max Health by 25% and an additional fight at the start of each level.
Veles also requires 700 Health instead of 500 plus you are tracked in the shadow realm (and you really don't want to get caught by them).
The Chef's Charm is key to this as it gives you 60% more Health from Food. Save your food and don't feed Veles until you have this.
Stack your deck with cards which give food. Play cautiously, avoiding the guards and using Food unnessecarily. Getting caught by a guard will cost you at least 25 Health.
If you have a weapon (such as Executioners Axe) which increases KOs for Veles to feast on then you'll save yourself some trouble.
Always use Ambush on Bounty Hunters as you'll get the 5 Health from the 1 Food you consume and if you're successful, Veles gets some Health too.
Good luck!

I am curious ... since this is a mask: do you need to equip it the whole time or just for the final fight to get the cheevo?

The Chef's Charm is great, but easiest way to get this is with the Crown of Scales, unlocked by getting the token for The Pit. The Crown of Scales halves damage from pain cards when your life is below 50% - including the damage you take when feeding Veles! So the requirement effectively drops from 700 to 350.
I ended up winning with Gladiator's Visage, Brigand's Shield, Rola's Last, Chef's Charm, Crown of Scales (which I usually just carried) and a bunch of other random stuff. Rola's Last (or even Sword of Light) + Brigand's Shield is better than the Harvest Blade for this stage IMO b/c of the large number of counters available.
Another absurd card, if you have the Outlands & Outsiders DLC, is The Silver Man. Flipping all your gold or fame to food can instantly give you enough food to stuff Veles like a piñata.
Finally I disagree with the recommendation to play cautiously. From the second stage on you can punch the patrols in the face whenever they're in your way. Getting all your gear from encounters is absolutely worth the 25 damage to heal Veles, and if you have the right gear you'll more than make up the damage. Just watch out for the shadow patrol - it's really, really rough.
Happy hunting!

The mask is called "Adherent Mask". Given for collecting a token consisting of 6 shadow shards.
The last fragment was taken for the task “Return to Bald Mountains”. There you need to take out all the enemies in a minute. No weapon will help since you are given only a basic set for battle. So you need to pump Veles to a state of bloody madness and start completing the task with him in this state.
To complete the "Servant" DLC with a mask you need to give 700 blood, so we stock up on food cards
