Complete all Underworld Encounters.
There are 4 Invasion from the Underworld cards. You get the first on automatically by finishing a game after the release of the Title Update 2. They are easy to recognize because of the red banner and icon in the top right hand corner of the card.
Quick Guide to Underworld Cards:
Indecent Arrival: Follow. Interrupt. Kill the Hell Beasts.
Alicia in Underworld: Pay 3 gold for a 25% success chance (You can repeat this). Give Artefact in your possession. Offer to Find Mystics. Kill Hell Beasts.
Greenborough Forrest: Pass 50% Success chance. Chop (You must have a strong weapon in your possession). Move to Underworld Totem card. Give 3 rings.
Descent into the Underworld: You must win a series of battles where you need to keep returning to the card to face the next battle.
Battle 1: Hell Beasts
Battle 2: Hell Beasts and another Enemy card (I saw Minotaur or King of Scales here)
Battle 3: Hell Beasts and a Lava Golem
Battle 4: Hell Beasts, 3 Lava Golems, and a Lich

Where does the the artefact come from that is required for the Alicia in the Underworld encounter? Might be good to include that in the solution :)

Just a short tip for descent into the underworld:
Although playing on apprentice makes it easier, this is no guarantee to finish it easily.
What i did is use the Wanderer Fate from the wildcards DLC, this allows you to move to allready discovered tiles without using any food for the move.
For weapons, i prefer the blood crescent sword since it can instantly kill (for a small portion of your health) any enemie AND you instant kill enemies on the ground with a downward strike (good for the small creeps).
Every other equipment is not specifically necessary, however i want to write down a certain combination of a curse and a ring that basically makes you invincible.
If you have the midas appetite curse active (where you consume money instead of food) and the emergency ring, you can enter a combat and as soon as your health reaches zero, you are completely healed to the amount of money you own. This goes on infinitely, so you are basically unkillable.
This combination can aid you in ANY hard encounter, however it is luck based as well

Just to add to the above solution if you are struggling with the final card descent into underworld then switch the difficulty to Apprentice, like all token cards Apprentice does not affect the token or this particular achievement
