From the main menu, press

to switch to the character tab, then choose Character Customization. From here choose any type you wish to change. Pick a preset and be sure to change everything.

You will have to change the following:
Gender
Voice
Hair
Hair Color
Eye
Eye Position
Mouth
Mouth Position
Skin

Some people report that changing gender isn't necessary, but I changed it just to be safe

From there hit

and be sure to save your changes by picking Yes. This will unlock the achievement.

I had to do it twice as described here and it popped. The first time it just didn't save the changes. I did choose Yes however.

I just unlocked multiple other achievements in the game but now trying this a bunch of times on multiple characters and nothing happens. Any suggestions from anyone?

Edit: Finally got it after having to constantly go in and out customizing the same character over and over again.

I had to push the right stick in to change the colour for this to pop

I changed 1st option on all classes, then a warrior 2nd with +1 change on all parts. Then the achievement popped.

This achievement won't unlock for me despite customizing everything to a character.

I wasn't able to get this achievement to pop until I changed each customization option on all 3 classes (warrior, cleric, and mage).

It wouldn't pop for me until I used the character in the first slot. None of the alt slots worked.

Mine worked with alternate the 1st time. This was obvious but glad someone posted a helpful hint tip. Always nice to have a play by play to avoid mistakes. Thanks for posting the advice. Best of luck to all.
