How to unlock the Finally, Vacation Time! achievement in Have a Nice Death - Definitive Guide

Finishing the game requires that you beat the secret end boss
In order to unlock this boss, you have to obtain all 3 of the pieces of the broken medallion in the same run. They are found in lockers that show up after you hear characters start talking about strange lockers, sometime after you initially beat Time.
They show up more than once per run (the shop one for example will show up in every shop).
They are as follows:
- Shop (Requires 1 Prismium to unlock)
- Breakroom in between departments (Requires 3 Golden Anima to unlock)
- In the hallway just before fighting any Thanager
I suggest doing it on the easiest difficulty since you have to put a fair amount of resources into getting the medallion pieces.

Who or what is Time? I've defeated the Major for the first time yesterday. Then hell broke loose and I died too, but the option for a second contract did unlock. The unlocking requirement was beating the game once. I'm confused here.

First time you beat the game, it ends at Major. Second time will end after beating time - there will be another department after Modern Warfare/natural disasters.
In runs after that, the lockers will start appearing.
