Edit 11/01/2019: With the release of the 1.10.33.0 update all the skins related achievements have been fixed. If you have already completed the requirements the achievements don't pop up right away, I had to apply a skin for them to unlock.
Currently unobtainable. The developer is aware and will try to fix it with an update.
You can buy skins with in-game currency that you earn with daily bonuses or resolving puzzles.
You can also buy a pack with all the skins with real life money.
To access the shop you need to be in a level and then click the shopping cart in the top-left part of the screen.

how do u even change or buy a skin, there doesn't seem to be an option on my screen, and if u choose the t-shirt u get rederected to the website, thnx

When you select an item there's a red button that says 'Buy', and when you select an item you own there's a blue button that says 'Apply'. If by t-shirt you mean the skin packs, when you click any of them a Microsoft Store window should pop up to finish the transaction. Maybe the shop isn't fully available in you region at the moment.
