The scope/pistol sight you will find in a wall locker in the third area just north of the shower room(not to be confused with the bathroom earlier on.) Pass code is 796
The power pill is found by entering the masks on to their mirror opposite to unlocking the door. Top row put Flame on the left and Leaf on the right For the bottom row put Feather on the left and Fishtail on the right.
IMPORTANT You must have both items in your inventory for the achievement to unlock. Since the scope will likely be the first item you find, DONT EQUIP it to the pistol. Instead, store it in the box or keep it with you.

I can confirm that you'll need to refrain from attaching the Pistol Sight to your gun in order to unlock this achievement. Instead, simply deposit the Pistol Scope into the item box after getting it from the wall locker. You can take it back out once you're ready and able to pick up the Power Pill.
Later, once you have all 4 correct Masks, place them on the mirrors appropriately to open the secret passage. Make sure you have the Pistol Scope in your inventory, then walk over and pick up the Power Pill; it won't be glowing so just mash A-button in the area.
I guess I wouldn't say the achievement is buggy. It's simply just programmed to unlock once you have both singular items in your inventory; attaching the Pistol Scope to your gun effectively removes it from the game's tracking and voids the achievement for that playthrough (requiring a save reload or New Game). So you'll need to have them both at once (without using them) to get the achievement. That's how I unlocked it anyway.

Didn't pop for me. However, the achievement for collecting all masks didn't pop either, I had to have them all in my inventory to make it pop. Could be a similar thing!?!?
