All right people, let's do this.
Just got this one, so here's how I fared.
1. In the convenience store, do NOTHING. The mugger will shoot the storekeeper. So that's one witness down.
2. Keep a close eye on everything that everyone touches in Manfred's shop. It's always going to be the music box, the door, the whiskey glasses, both phones and the magnifying glass. But you might do things differently. So just pay attention.
3. Jayden died for me in Pacos office. I'm not sure if just dying works, because it clearly says "KILL Jayden" So, I just didn't fight back in that fight.
4. Scott went straight for escaping and didn't try to free Lauren. Cold, I know, but she was an Origami witness.
5. Don't give Kramer his pills. Simple as that.
6. Now, Madison, i just didn't put out the fire when her pants caught fire. As such, she died writhing and screaming on Origami's bed. Again, the trophy description says Kill Madison, so I don't know if Dr Baker would count.
And lastly, I completely 3 trials, got the correct address and never had Ethan arrested. As such, he managed to save Shaun and then was shot by police snipers.
That's just what got me the trophy, though. You could theoretically have Jayden die at the end, or have Madison caught in the explosion. Not sure of the effect of Ethan in gaol (jail) though...

Instead of a fire or explosion you could also let Madison die by jumping out the window in Killer's Place.

This solution and RossWyatt's solution will work, but they fail to mention that the game must be played through completely, either from the start on your first playthrough, or from Hassan's Shop if you have completed the game.
I continuously would not get this as I thought I could just load the specific chapters and save.

Borrowed from a guy named "_Shuyin_" from GameFAQs. His solution solved this for me so simple that I had to put it up on here. I take absolutely no credit for this what-so-ever, just trying to make the guide known to people on here.
Hassan has to die in Perfect Crime. Doesn't matter if you retrieve the shoe box or not. As long as he's dead, you're golden.
You can get perfect crime and the ending where Ethan commits suicide in prison on the same playthrough (I can confirm this as I've done it).
Also, you don't need Ethan to get arrested twice. You can skip the first one and let him get arrested at the motel. Since Jayden should be dead by then (he needs to die for the Perfect Crime run) Ethan will get locked up.
To get the Perfect Crime, follow this:
1) Hassan dies in Convenience Store by the robber. And thats ALL that needs to happen at this part.
2) Let Dr. Death kill off Madison.
3) Let Mad Jack kill off Jayden
4) Clean all of the prints from Manfred's Shop.
5) Let Ethan get arrested at the motel.
6) Let Lauren drown
7) Take Kramer's pills and walk out the door with them.
The game should end after Scott burns the evidence, shortening the game for you.

There is one very important nuance in Manfred's house!!!
