Upon finishing your trek to the farthest reaches of the mortal lands, you'll find your destination being the large bridge leading into the Land of the Dead, Helheim. Your trophy should be awarded after the brief instance you interact with the Northmen found at the gate awaiting you before the bridge.

This achievement is story-related and cannot be missed.

Towards the start of the game, you will eventually reach a beach and then scale a cliff. Soon after, a puzzle door will block your path. The achievement will unlock upon completing the puzzle and venturing towards the gate that leads to Helheim.
