Using the key from day 4 unlock the upstairs back right door. Inside the room open the book to collect a wrench, backtrack out of the door, past the bear and open the locked door beside it with the wrench. Collect the book on top of the toilet and return back to the first room we unlocked. Place the book on the bookshelf and interact with three books that are targetable. This will reveal a bear head, which we simply need to place on the bear statue outside this room for the achievement to pop.

I got the achievement by following the instructions here. However, I think the intent is probably to use a different head (like the fish head) on the bear. That's what the achievement picture shows. This will likely get fixed in a future patch.

This is completed in Night 4. Use the key you previously obtained to unlock the final locked door on the second floor. Grab the Tome from the bathroom and place it in the right bookcase of the newly unlocked room. Then interact with the book you just placed, as well as, the red book above it, and the green book on the left bookcase. Grab the Bear Head and place it on the Bear Statue just outside of the room.
