Most easily accomplished on the first mission. Set the difficulty to 'Rookie', and be very, very careful. When attacking the bombers, even on the easiest difficulty setting, DO NOT EXPOSE YOURSELF TO THE TAIL GUNNER'S FIRE! Also, beware the final Messerschmitts. Take out the leader first, (the one in the middle), and the accuracy of those remaining will drop significantly!
Overall, the first mission should present few problems with regards to this achievement. Later missions are much more difficult to even attempt to unlock this achievement on!

You can be hit by enemy fire, you cannot however loose one health bar.

It's also worth mentioning that you can crash and reload your last checkpoint without having to worry about missing this achievements.

You may get this just by playing through the campaigns on Pilot difficulty, since your health regenerates to an extent. If not, start up a mission on Rookie and play through the level until you get to the last checkpoint. Don’t worry if you get damaged; on Rookie, you can reload a checkpoint and your health will be fully restored. Complete the mission without being damaged.
The best mission to get this trophy is mission 3, London Burning. You can literally beat this mission without killing anyone. Simply fly around, avoiding enemies, until the mission is over.

Finish a mission on full health; very easy to understand, very easy to do. The easiest way of doing this is probably the first mission on Rookie difficulty, due to the fact there are very little planes, thus, very little bullets. Just make sure your health bar is full when you finish the mission, and this achievement is yours.
