During the boss fight with QA-1MIL he'll eventually start throwing downward punches that get stuck in the floor - leaving them vulnerable to attack. Do enough damage to the hand and it'll open up. Once open you'll be prompted to press 
to start a brief quick time event that has two timed button presses. If hit perfectly you'll throw QA-1MIL's hand back, hitting himself in the face. Repeat this again for a second time and it'll expose a weakness on his head. The achievement should pop during the brief exposure cutscene.
If you manage to defeat QA-1MIL without triggering the two quick time sequences. I believe you can return to Quality Assurance when stage select becomes available later in the game.

I’m sure you can get it on phase two as well as I did half his face when you started and the other half when he had red armour on

During the first boss fight in the game against QA-1MIL, his attack patterns will start off with swipes and then a fist slam in his first phase. His fist will get stuck in the ground allowing you to beat it up. Do enough damage in these instances to trigger a QTE. Press the timed buttons to the beat to make the boss punch itself in the face. This will reveal one half of its face. Do this again to reveal the second half and unlock the achievement.
His second phase is similar with swipes and fist slams; however, his combos are longer before he is open for damage and he has an added set of cannons that he fires occasionally. You can earn this during the first or second phase, assuming you don't fail the QTEs.
Levels can be replayed through Level Select, which unlocks through story progression.
