A beat-hit is the quick time event that appears at the end of a successful combo. As the two circles overlap simply press either the 
or 
to complete a beat-hit. This achievement requires you to do 20 which you'll likely do in the first stage, then you'll need 500 for the Beat-hit mania achievement.

Note: You can turn off "low latency commands" in game options to improve your combo window by "decreasing game input precision" (just not worked for me through Xcloud, even having a VERY LOW lattency with the servers).
Instead of keep smashing buttons like in DMC (specilly playing as V in the 5), GOW and other hack&slash games, Beat Hits get even easier to do waiting the actual attack to hit and THEN hitting the button for the next, almost like the combat flows in Batman Arkham series. It should keep your inputs in a rythm and you'll just need to care with the QTE for the last hit to complete the job.
Like Lordacris said, you'll should see it popping in no time doing everything right - even being a clumsy no rythm at all player like me.

Achievement didn’t pop but it did show the achievement when I went under the title for achievements on console and on trueachievements.
