This occurs after you collect 4 life gauges, which should occur naturally around the 3rd or 4th stage as you play through the game.

micmon83, you might have to find another life gauge. I was able to unlock this after getting rid of the pigeons in the second level that is a part of the I am a good person who likes to help achievement

Can you just buy 4 upgrades from peppermint?

Life gauges bought from Peppermint do count for the achievement. It's been a few months since I've played so I can't remember how many she sells, but I do remember that they're expensive so this probably isn't the easiest way to get this achievement.
