Just wanted to add that the achievement unlocks immediately after the QTE section is completed (so, well before the subsequent cutscene completes).
Also, for the ones where you need to hold the button, if you release it at the correct time, there should be a sound confirming that it was released correctly (if you don't hear the sound, then you've held it too long).
