Hard to miss.
Through normal progression, Knifey will get his chainsaw upgrade. And a few minutes later a Bloat (enemy with two heads) will appear.
Just keep knifing this Bloat until the execution.
If you mess up on the first Bloat you see, many more will appear anyway.

I've been having problems with pretty much all of the DLC achievements, but this one is being particularly stubborn. Hard to miss (like you said), yet it hasn't unlocked after 10+ bloats
