To do this challenge you need to find the special easter egg, if you can call it that.

WARNING THIS TROPHY IS VERY SPOILER HEAVY, DO NOT READ UNTIL YOU HAVE FINISHED THE GAME, THIS LEVEL ALSO CONTAINS SPOILERS FOR HITMAN 1 AND 2 AS WELL AS THE ORIGINAL TRILOGY IF YOU HAVE YET TO PLAY IT.

At the end of the game, grab the syringe from the briefcase and hold it for 47 seconds. A button prompt will appear to use it on yourself, do so and unlock the secret ending and your achievement / trophy.
